Secure Motability Eligibility Portal

To process your Motability application and arrange your equipment, we need a few details to run a secure eligibility check. Your information is encrypted, strictly confidential, and processed in full compliance with GDPR

Section 1: Contact Details

How we keep you updated.

We need your basic contact details so James or Ant can reach out once your eligibility check is complete. We will only use this to update you on your application status or to arrange your handover—we promise we will never share your details or send you spam.

Section 2: The Secure Details (DOB & NINO)

Verifying your eligibility.

Because the Motability Scheme is linked to your government mobility allowance, we are required to securely submit your Date of Birth and National Insurance Number. This allows the system to communicate directly with the DWP or Veterans UK to confirm your eligibility.

 

Your data is fully encrypted, treated with strict confidentiality, and never stored on our public website.

Section 3: The Address Block

Where will the equipment live?

Under Motability Scheme rules, your new scooter or powerchair must be registered to the primary home address of the allowance recipient. We also need this to organize your delivery, home demonstration, and future servicing.

Before You Submit: Important Information About Your Lease

Please take a moment to read and confirm the following terms, conditions, and features of the Motability Scheme lease agreement:

  • Ownership: Your new scooter or powerchair will be owned by Motability Operations throughout the duration of your lease.

  • Who can use it: The equipment is for your exclusive use. If it is driven or damaged by someone else, it will not be covered by your insurance.

  • How payments work: Your lease is paid directly from your mobility allowance. These payments are sent straight to Motability Operations by your benefit provider.

  • If your allowance changes: If your mobility allowance is stopped for any reason during your lease, the equipment will need to be returned to us at MLV Mobility.

  • What's included: Your insurance, regular servicing, maintenance, and breakdown assistance are all fully covered in the lease. The only thing you would ever need to pay is an insurance excess if you have to make a claim.

  • Cancelling within 8 days: If you wish to cancel your lease within 8 days of receiving your scooter or powerchair, you must contact Motability Operations directly.

  • Ending the lease early: If you choose to end your lease before your contract finish date, Motability Operations reserves the right to charge a cancellation fee.

  • Your Data & Privacy: By submitting this form, you agree that Motability Operations and the Motability Foundation will securely collect and share your personal information to check your Scheme eligibility and create your Motability Scheme online account, strictly in line with their respective Privacy Notices.
